Masters Programs

M.S. Degree Completion Procedures
This link provides access to the Graduate School web page detailing schedules and procedures for filling out necessary forms.

M.S. Graduation Instructions
This form indicates which additional forms need to be completed before graduating.

M.S. Full-time Status with One Credit Registration Application
With this "Advanced Master's Status" form students become eligible to take a reduced courseload through the FTE (8333). Eligibility requirements:
1. An approved master's Degree Program Form is on file with the Graduate School.
2. All coursework completed included on the Degree Program Form, with grades posted to the transcript prior to the term of application.
3. If Plan A, met the master's thesis credit requirement (10 semester credits of xxxx 8777).
4. If Plan B, completed all coursework and *only* the Plan B project is not graded.

M.S. Degree Program Transmittal Form
This forms outlines a student's coursework plan to earn a degree. This form should be submitted after the second semester of work. It is imperative that students fill it out early in their degree program as it will help determine what classes are needed for degree completion. The form also lists members of a student's defense committee. This form can be modified/changed using the Petition Form if needed.

M.S. Plan C Tracking Form
This form is required for all Plan C students to track their progress and completion of program requirements.

M.S. Graduation Packet Request
Students must request this graduation packet in order to graduate. Packets include forms such as Commencement Approval, Final Examination and Graduate Application for Degree forms.

M.S. Commencement Attendance Approval Form
Students intending to attend the Graduate School Commencement Ceremonies and have their names in the program need to fill out this form and return to the Graduate school. For Fall Commencement the form must be submitted by Oct. 1 and March 1 for Spring Commencement.


Doctoral Programs

Ph.D. Degree Completion Procedures

Ph.D. Graduation Instructions
This form indicates which additional forms need to be completed before graduating.

Ph.D. Preliminary Oral Examination Scheduling Form
This form must be on file with the Graduate School at least one week in advance of the Preliminary Oral Examination.

Ph.D. Final Oral Examination Scheduling Form
This form must be on file with the Graduate School at least one week in advance of the Final Oral Examination.

Ph.D. Preliminary Written Examination Report
This form indicates whether the Preliminary Written Examination was passed or failed.

Ph.D. Thesis/Project Proposal Form
This is a three-part form that confirms your major field panel and officially submits your thesis proposal to be on file at the Graduate School. This form should be submitted after passing preliminary examinations.

Ph.D. Full-time Status with One Credit Registration Application
With this "Advance Doctoral Status" form students become eligible for a pay raise and to take a reduced courseload through the FTE (8444), and thus it is important to fill it out as soon as you become eligible. Eligibility requirements:
1. Successfully completed the preliminary written examination.
2. Successfully completed the preliminary oral examination.
3. Have a signed examination form on file with the Graduate Student Services and Progress (GSSP) Office.
4. Completed all coursework included on the Degree Program Form, with grades posted to the transcript.
5. Registered for 24 doctoral thesis credits.
6. Ensured the appropriate student-group designation appears on the academic record (Students can verify this with Peg in the accounting office).
